Previewing your organization's New Vendor Registration Form or Short Form helps you assist vendors by showing the fields and information they may need to provide.

Steps to Preview Your Registration Form

  1. Log In to PaymentWorks

    • Ensure you are logged into your PaymentWorks account.
  2. Access the Form Preview

  3. Review the Form

    • The registration form should load, allowing you to preview all fields and instructions.
    • Note that some fields or questions may vary based on the vendor type or how the vendor responds to specific registration fields.

What If You Cannot Access the Form?

If the link does not allow you to view the form, it may be due to insufficient permissions. To resolve this:

  • Contact your organization’s PaymentWorks Administrator to request the necessary permissions.

Short Form Functionality

The Short Form functionality is available upon request. If your organization is not yet using the Short Form and you’re interested in enabling it:

  • Submit a request to your PaymentWorks Administrator or Support Team for activation.

Once enabled, use the appropriate link above to preview the Short Form.
